CT- Virtual Colonoscopy/Endoscopy

Colonoscopy, fecal occult blood testing, sigmoidoscopy, and barium enemas are well-known and familiar screening tools that are utilized in the early detection of colorectal cancer. Unfortunately, compliance with colorectal screening is very low. Figures as low as 15% of the age-appropriate population have been reported, as representative of those who routinely undergo any type of colon screening at all, with smaller percentages actually proceeding to colonoscopy.

Many patients loathe the idea of being sedated, and are equally dissatisfied with undergoing an examination that requires the introduction of a diagnostic instrument into their colon. Since colorectal cancer is largely preventable with the identification and removal of adenomatous polyps, "Virtual Colonoscopy" seems to be the way to go; it is certainly an appealing alternative that will encourage a higher patient participation.

Virtual Colonoscopy or CT Colonography, utilizing 64-slice CT and 3D reconstruction, is an excellent method to screen for colorectal adenomas, which are the precursors in the majority of colorectal cancer patients. It is a less invasive and more comfortable screening procedure than optical colonoscopy. Virtual colonoscopy requires no IV sedation, but does not spare the patient the preparation process.

If pathology is found, the patient can then proceed to optical exam and polypectomy, which can be performed more efficiently with the guideposts established by the CT scan. For the majority, among whom no pathology will be found, the virtual exam offers a much better option.

Excellent virtual colonoscopy is dependent on four critical points: Optimum bowel cleansing, maximum bowel distention, state-of-the-art hardware and software, and dedication and experience on the part of the imaging center.
